Case Management

Suffolk County United Veterans (SCUV) manages 9 independent residential houses throughout the County.  The mission of our Veteran Housing Program is to help regain entry into society and attain residential stability, self-sufficiency, and economic independence. It is achieved by providing appropriate permanent housing for Veterans, as well as by establishing linkages with local service providers to obtain information, resources, and services which will empower homeless veterans to facilitate their successful reintegration into society and remain housed. Connections are also made to physical health, mental health, substance abuse treatment providers with follow up and monitoring.  Our skilled Housing Case Managers are assigned to each client to monitor and respond to substance abuse situations, mental health crisis, physical health emergencies.
