Social Care Network Coordinator and Trainer
Salary: $72,800 per year- Exempt
Hours: 37.5 hours per week
Location: Travel throughout Suffolk County
Availability: Immediate
Qualifications:
- A Bachelor’s degree in a human service/health discipline and five years’ experience is preferred delivering direct care, 2 years of supervisor and 2 years of delivering training. Candidates with Master’s degrees are strongly encouraged to apply.

- Excellent inter-personal communication including the ability to establish empathic, empowering relationships with people who have experienced psychiatric, substance abuse challenges, trauma as well as chronic physical health conditions;
- Strong communication skills and collaboration skills;
- Understanding of the key elements of recovery and wellness
- Sound organizational skills including computer skills
- Basic knowledge of the public health/mental health system, public entitlement and social service programs;
- Demonstrated ability to communicate effectively verbally and in writing with people of different cultural and socioeconomic backgrounds
- Ability to complete required trainings and additional certifications or trainings as assigned
- Organizational and time management skills
- Ability to prioritize and demonstrate flexibility in day-to-day functions
- Candidate must be a reliable self-starter with the ability to work both independently and as part of an interdisciplinary team.
