Team Supervisor – Forensic Team, Adult Care Management Program

Salary: Based on experience and credentials, competitive salary and benefits package

Hours: Monday–Friday, 8:30–4:30 (2 office days required)  

Location: Riverhead, NY

Position Description:

This Supervisor works on a specialized team of those serving justice-involved members; utilizing effective approaches to engage members who were formerly incarcerated, fostering relationships that are free of the mandates these members are often subject to when incarcerated/on parole. This Supervisor works closely with parole officers, jail/prison systems and re-entry entities to assist members with successful post-release reintegration into their homes and communities. Key responsibilities will include:

 

  • Maintain Health Home Plus caseload of members with Serious Mental Illness (SMI).
  • Proficient in conducting appropriate screenings/assessments (i.e. HARP Eligibility Assessments, Risk Assessments).
  • Coordinate Care Management needs and successfully engage high need individuals by utilizing a recovery-oriented approach and skills such as motivational interviewing and trauma-informed care.
  • Conduct team meetings on the Team Coordinator’s behalf as requested.
  • Assist with the monitoring of team’s diligent search efforts to reengage clients.
  • Review/distribute reports and ensure that members of team are meeting service delivery requirements monthly.
  • Assists Team Coordinator with the oversight of special projects.
  • Track and Monitor team’s completion/submission of documents (i.e. HML, hospital alerts, etc.)
  • Collect and review team’s encounter logs prior to submission and sign off.
  • Assist in the tracking of inactive Medicaid list and follow up on status with team members.
  • Provide support to new team members as they navigate IT platforms (i.e. GSI/Care Tool).
  • Identify performance issues/training needs and bring to Team Coordinator’s attention.
Qualifications:
  • Master’s Degree preferred
  • 2+ years’ Health Home/State Aid Care Management experience
  • Experience serving formerly incarcerated/justice-involved individuals
  • Leadership experience preferred
  • Understanding of the key elements of recovery and wellness, including the role of family and self-help.
  • Candidate must be a reliable self-starter with the ability to work both independently and as part of an interdisciplinary team.
