Salary: $55,000 per year

Hours: 37.5 hours per week

Location: Ronkonkoma and Riverhead, NY

Position: PROS Clinical Practitioner

Salary: $55,000 annually, plus benefits.

Hours: Monday- Friday, 8:30am-4:30pm; one evening per week (specific per site), as needed.

Location: Initially this will be a Shared Full-time position, Ronkonkoma, NY.

 

Position Description:

MHAW is seeking a NYS licensed practitioner of the healing arts (LPHA) to join a team of Behavioral Health, Medical, and Peer providers at the Pollack and Synergy Centers, to assist the individuals we serve in pursuing Recovery and Wellness goals, through the provision of highly integrated psychiatric rehabilitation, treatment, and support services. The PROS Clinical Practitioner will report to the PROS Program Director for supervision and assignment of clinical interventions and service provision.

In addition to the provision of PROS and Clinical services, the PROS Clinical Practitioner will serve as an anchor for clinical services within the program. They will be a resource for the PROS team in developing and implementing responsive and evidence based interventions for Clinical Services members aligned to the Clinical Services goals for the organization.

Key Responsibilities and Tasks

  • Manage an assigned Caseload for optimizing PROS service delivery, and required program documentation.
  • Assist Clinic participants with establishing meaningful rehabilitation and recovery-oriented goals.
  • Conduct program Intake Screenings and Clinic Screening Assessments.
  • Provide individual and small group services, to identify at risk/high need participants.
  • Review and sign off for PROS Clinical Services on participant’s Individualized Recovery Plans.
  • Provide Clinical Counseling and Therapy (CC&T), a goal-oriented and solution-focused intervention, including individual, group, and family counseling or therapy, as indicated on IRP.
  • Daily direct-service productivity expectations will be established and required, based on the PROS program and Clinic component census.
  • Attend and participate in identified programmatic and agency meetings.
  • Collaborates and coordinates PROS service provision with the agency Clinical Administrator.
Qualifications:

The successful candidate should possess the following skills and abilities:

  • Master’s degree from a qualified, clinical Graduate program in a Behavioral Health/Human Service field; with an active NYS licensure in the following LPHA disciplines: LMSW/LCSW, LMHC, LCAT, LMFT, LPA; and at least 2 years post-Master’s and licensed experience.
  • Excellent inter-personal communication, including the ability to establish empathic, empowering relationships with people who have experienced psychiatric and substance abuse disabilities and trauma; Strong communication skills with health professionals is also required.
  • Experience with providing clinical counseling/therapy to participants with Co-Occurring (MH/SU) challenges
  • Service planning and record-keeping in a psychiatric rehabilitation, person centered practice and recovery-directed framework; sound knowledge of the Behavioral Health services system (OMH, OASAS).
  • Excellent documentation skills and demonstrated proficiency in use of an Electronic Health Record (EHR).
  • Excellent organizational, time management and computer/technology skills.
  • Proficient in the use of current technology: desktop applications, smart phone apps, MS Office 365, Zoom, etc.
  • Understanding and Commitment to Corporate Compliance; as identified in organizational and programmatic protocols, regulatory statutes (local, state, federal) and educational requirements.
  • Bi-lingual Spanish a plus; stipend available for verified language capability.
  • Commitment to racial/ethnic equity, inclusion, and diversity.
  • Ability to provide cultural and linguistically appropriate services.
  • Candidate must be a reliable self-starter with the ability to work both independently and as part of a team.
  • Valid New York Driver’s License and meets the criteria to be an agency driver.
