Peer Outreach Care Manager

Salary: $20.51 per hour

Hours: 18.75 hours/week, some flexibility for eve/weekend

Location: Riverhead, NY – travel throughout Suffolk County

Availability: ASAP

Position Description:

Peer Outreach Care Managers outreach to the community and establish supportive helping relationships with people who do not have Medicaid, or transition in and out of Medicaid eligibility, and have significant behavioral and/or physical health conditions. If an individual appears to be eligible, the Peer Outreach Care Manager helps to secure Medicaid benefits with the goal of enrollment into a Care Management program that will assist the individual in achieving improved overall health and social functioning. Key activities include:

  • Utilizing personal, lived recovery experience will be fundamental and expected in your Care Management role and practice;
  • Coordinating and monitoring behavioral, physical health, and social services based on consumer-driven needs assessment and service plan;
  • Building and monitoring linkages with natural supports and formal community resources including shelters, hospitals, outpatient clinics, rehabilitation, self-help, and social service providers.
  • Providing supportive, recovery and wellness-directed coaching and counseling in-person and via telephone communication.

 

Work is done primarily in the community and position entails travel throughout Suffolk County. Agency vehicles can be used but a safe, personal auto and a clean NYS Drivers License are required; mileage reimbursement provided. Peer Outreach Care Managers work in teams and receive weekly supervision and as much support as needed. Great working environment; laptops and cell phones provided.

Qualifications:

A Bachelor’s degree in a human service/health discipline and two years’ experience is required. NYS Peer Specialist Certification required (or completion underway). Bi-lingual (English/Spanish) candidates are strongly encouraged to apply

Ø  Excellent inter-personal communication including the ability to establish empathic, empowering relationships with people who have experienced trauma, psychiatric and substance abuse challenges as well as chronic physical conditions;

Ø  Strong communication skills with health professionals involved in the consumer’s care;

Ø  Understanding of the key elements of recovery and wellness, including the role of family and self-help;

Ø  Sound organization skills including computer skills (agency uses electronic health records).

Ø  Basic knowledge of the public health/mental health system, public entitlement and social service programs;

Ø  Candidate must be a reliable self-starter with the ability to work both independently and as part of an interdisciplinary team.
