Administrative Outreach & Engagement Team Member

Salary: $40,000 annually ($20.51 per hour (non-exempt))

Hours: 37.5 hrs/week; 8:30am – 4:30pm (some flex eve/wknd)

Location: Ronkonkoma, NY

Availability: ASAP

Position Description:

Responsibilities:

  1. Completion of all data entry tasks, in Cerner and/or GSI/ CareTool, to maintain information associated with potential and enrolled clients (i.e. adding and/or updating demographic information and client assignment).
  2. Completion of Progress Notes in GSI and CareTool to reflect all outreach and enrolled activities (i.e. phone calls, internet searches and mailings). Process closures and manage patient programs as required in Cerner and/or GSI/CareTool.  Verify Medicaid eligibility via ePaces.
  3. Data entry and/or uploading of assessments and other completed documents for prospective and enrolled clients into Cerner, GSI, CareTool, and other data programs/IT platforms as requested.
  4. Ensure that clients’ records are accurate and complete (i.e.: includes all assessments, authorizations for disclosure of protected health information, demographic/insurance information, backup documentation supporting HH eligibility, etc.)
  5. Accurate completion, audit and submission of daily encounter logs to reflect completed outreach and enrolled activities. Engage in regular communication with O&E Care Manager(s) as prospective clients express interest in the program by phone.
  6. Respond to incoming phone calls from prospective clients and reply promptly to inquiries, transferring to O&E Care Manager(s), or Outreach Team Coordinator when appropriate.
  7. Assist clients with linkages to services in the community, including physical and mental health providers, treatment centers and supportive community resources, as appropriate to the client’s expressed needs during outreach and enrollment.
  8. Inform newly enrolled client of his/her enrollment in our Care Management program and his/her upcoming assignment to a Care Manager or recently reassigned client of his/her new Care Manager.
  9. Relay any immediate concerns/needs and/or updated pertinent information regarding prospective or enrolled clients to the appropriate supervisor.
  10. Assist in the completion of additional projects/data entry tasks necessary to support other departments and programs as requested.
  11. Assist in the distribution of keys for and the maintenance of the Care Management program vehicles.
Qualifications:
  1. Candidate must have at least HS diploma, but college degree is preferred
  2. Proficient in Microsoft Office (Excel, Word, etc.), familiarity with entering and analyzing data in various IT platforms
  3. Customer service/experience on the phone (i.e. telemarketing, engaging customers, high volume calls); Spanish speaking preferred.
  4. Candidate demonstrates initiative, is a self-starter/problem solver who has ability to multi-task
  5. Attention to detail, editing/auditing records
  6. Skill and sensitivity in communications with those experiencing mental health and physical health challenges
  7. Team environment – must work well with others
